And a triangle allows for gradations of importance in 2-D and the pyramid in 3 D. The bottom is big so you put the things you want the people to do most of there and put the things that should be used rarely (such as drugs and surgery in Cowen's) at the point. Circles squares and octogons are going to give impression of equal emphasis. you could use concentric circles , but it would be confusing and then if projected it 3d to help clarify, you'd have a cone which is back to kind of like a pyramid. dodecohydron anyone? or how about a nonagon?
