You're absolutely right, it is the t2201. It was a 1 page form that

usually is included in everyones tax form package. I filled in most

of it and the doctor put a few notes along with his signature. It

was really easy. We did this in 2003 I think and we received our

benefits retroactive to the year he was born. It is worth the effort

to make an appointment with a pediatrician(bring the form with you!)
