Research Study to Evaluate Family Quality of Life

You are invited to participate in a research study to evaluate family quality of

life in

families with young children with autism spectrum disorders. If you have a child

under the

age of six years who has been diagnosed with an autism spectrum disorder, please

consider participating in this survey. Your participation will help the

investigators identify

the unique needs of families with young children with autism. The survey will

take

approximately twenty minutes to complete and you will have the option to

complete

additional surveys as part of the same study at the end. The additional surveys

will take

approximately forty minutes to complete.

Please only complete the survey once.

Survey participants must have a child under the age of six years who has been

diagnosed

with an autism spectrum disorder.
