Insurance ABA 1099

I have UHC for insurance and I submitted my ABA(provided by someone who

is not a BCBA) bill to insurance and isnurance is aksing me that I

submit a 1099. Do I ask the provider to fill in the 1099 or do I give a

1099 to them? How does this work - sorry for asking very silly

questions but this tax thing is maze for me.

Any help is greatly apprecaited.
