In off-list correspondence with Marla, I came up with the following: The problem in question isn't Auspitz's sign, however; that appears in the very next paragraph as a separate issue. There is a three-point list of disorders caused by scratching - first, the " candle phenomenon " , where mild scratching creates flakes that look like candle wax; second, the " last piece of skin " ; and third, the Auspitz phenomenon, where the base of the skin is broken through, leading to punctiform bleeding. I think my original contextual quote wasn't broad enough. Looking at the above paragraph, it is clear that the second of the complaints describes the last piece of skin as _still being intact_ (all other layers having been removed); when that piece is perforated, the result is the third stage, i.e. the Auspitz phenomenon.
