Dx- diagnosis

Hx- history

Px- physical examination

MMR- Measles Mumps Rubella

DPT- Diptheria Pertussis and Tetanus

CHF- Congestive Heart Failure

CC- Chief Complaint

Ca- cancer or calcium

resp- respirations

COLD- chronic obstructive lung disease

COPD- chronic obsructive pulmonary disease

ER- emergency room

ED- emergency department

C+S Culture and sensitivity

q.i.d. four times a day

b.i.d. two times a day

d and c - dialation and curretage

ng- nasogastric tube

perrla- pupils equal round reactive to light and accomodation

NPO- nothing by mouth

NYD - Not yet diagnosed

TURP- transurethral resection of the prostate

TIA- trans ischemic stroke

CVA - cerebrovascular disease

CT Scan- Computed Tomography Scan

Fx- fracture

Rx- prescription

MRI- magnetic resonance imaging

PH- past history

More Will follow eventually. These are just off the top of my head from the

ones that I have studied through Medical Terminology.
