Since 1991, I have kept a single sheet of paper, that I call

my " medical resume " . It lists:

my full name

the date

my current doctor/s

any medications I may be taking

any vitamins/supplements I take

allergies

diagnoses

EMG/NCV studies

hospitalizations/surgeries

physical therapy I've had

and birthdate, deceased/living family and Advance Directive info.

I review this 2 times every year and update it as needed. I decided

to do this first for my own files (as a patient), so I could have

instant access from my past, for the present. Secondly, this is

helpful when/if I start seeing a new MD. It helps me communicate more

effectively with the doctor andd vice versa. This 'tool' has been well received

by my new docs since my last move in '98 and change of docs.

One told me " I've never seen anything like this before, I wish all my

patients would do this. "

And yes, it goes all the way back to 1962 when I was diagnosed with CMT.
