Guest guest Posted May 20, 2008 Report Share Posted May 20, 2008 I just received a follow-up letter in the mail asking for me to fill out a " Daily Activity Questionnaire " and a 7 page form asking for my work history for the 2nd time. When I called the Dept. of Assistive and Rehabilitative Services and spoke with the woman that sent me the form, she was rather rude. I asked her why she didn't have my original paperwork that clearly had my work history included, and she " barked " at me, stating that they weren't the SSA and didn't have all of that info...yet, she is the one that is determining whether I will get disability or not???? Hmmmm.... I told her that I could barely write and in an effort to avoid redundancy, I would call and get her another copy of my work history as well as proof that I have paid into the SSA for the last 15 years of steady employment before being diagnosed with CMT. Anyway...my question is this...I filled out this " Daily Activity Questionnaire " , but wondered just what they were " looking for " ....it is clear that they are looking for anything to deny disability. Have any of you filled out this form and then been denied disability? It is optional, but I didn't see any harm in filling it out. Also, have any of you been asked to have a " Consultative Examination " ? What is that all about? Do they expect for you to look really BAD to get disability?
