Congratulations. I’ve been using

Praxis since starting in June 2004, and would be happy to answer any questions,

discuss my experience, etc. I’m really happy with it, but have had a few

growing pains in terms of hardware, etc.

Newbie, Naming, and Discounting

Hello everyone!

I've been lurking and absorbing for about 6 months, and my practice is

starting to take shape, so I thought I'd start joining in. I'm up to

spring 2005 of the archives; I'll try to avoid too many repeated

questions.

I finished FP residency in Asheville,

NC (hello to the western NC

folks!) in 2003. I've been medical director at a small CHC in

Portsmouth, NH until recently. I've moved to the NC

Outer Banks (hello

to the southeast VA folks!). I hope to get my " solo-solo " practice up

and running by the spring.

Many, many thanks for all the great information and encouragement I've

already gleaned. I had been thinking about this sort of practice for a

long time, but you all put the real-life pieces together for me.

Particular thanks --

-- to Gordon , of course, for starting it all.

-- to Wes Bradford and others for turning me onto Praxis EMR. I've

signed on but will try not to be smug.

-- to Brady for a good practice naming idea. I've about decided

to leave " medicine " and " health " and " center " and

" clinic " out of my

practice name and be " One Family Doctor " , since that's really all I

am. I have several reasons for using " One " , but I'm looking for

objections to using this. Too goofy? Anyone?

I won't go into much more practice detail for now, but I do want to

ask if there are any new perspectives on discounting for cash-paying,

non-Medicare patients. It's been a while since I've seen it discussed

in the archives. I plan to accept Medicare, Medicaid, a few common

insurances, then offer a SimpleCare-type alternative set of time-based

charges also. I will provide a hefty discount for payment at time of

service, as well as a sliding scale discount based on ability to pay

(percent of poverty level, for a limited number of patients). I know

that there is some legal risk to this, and I am willing to take that

on. But is there any new information that I haven't found on this issue?
