Questions about CPTs and ID #s

I was wondering if anyone had some good ways to assign patient record numbers. Do you embed a date, simple number based on their order of establishment into the practice, or some other method?

I also wondered if anyone knew how to get a hold of a list of the most common (25-50) CPT codes an FP uses in practice. I would like to review these so that I can then refer to Medicare rates for my area in order to more thoroughly set up my fee schedule.
