I guess your first step would be to find out what the " Medicare

guidelines " are that are the basis for your denial. I doubt your

insurance will tell you, so here is something I found that may help:

http://www.medicare.gov/Contacts/Home.asp

It is the GOVERNMENT'S medicaid website and this will give you state

contacts to ask questions. You can also find other helpful webpages

and there is a publications list which you can view or order. Maybe

you can contact someone to steer you in the right direction. I have

a feeling that there are a ton of publications to dig through (not

unlike the IRS), and someone in the Medicaid system should surely be

able to help!
