I think they may be what your are looking for: AMA resolution 119p1 and p2. I think Polly Janos may be of some help if there is more that you need. She hasn't posted in awhile, but I'll bet you could try to email her directly. Hope this helps some. They will not look it up and may read it if you provide it for them. My appeal was a two page letter with 15 references attached...felt like I was back in school doing a term paper. I am still awaiting their answer. If they deny again, I will probably appear in person with my baby to appeal. Good luck and hope this helps.
