I plan on using this information to lobby for change in government regulations in nursing homes. If you have ever worked, interned, or volunteered in a nursing home (skilled nursing residential facility) would you please take a few moments to answer these questions and email them back to me? Please feel free to post this email or any other listserv that is available to you. I am interested in hearing from anyone who has had experience in this area. Nursing Home Survey 1. Are you now or have you ever been employed in a nursing home? 2. Have you ever interned, volunteered, or done field work in a nursing home? 3. With what department are you working? (Recreation, Social Services, Activities, etc.) 4. Where you hired full-time, part-time, per diem, or as a consultant? 5. What was your title? How were you described on your ID badge? (Recreation Therapist, Drama Therapist, Music Specialist, etc.) 6. Were you required to join a union? Which union? 7. Have you ever worked in a supervisory or administrative position in a nursing home? If so what was your title? 8. If you worked in a supervisory or administrative position, how many years or experience did you have working in a nursing home before you were hired in this leadership position? 9. Are you credentialed in your field of creative arts therapy? What is your credential? Are you licensed in New York State? What degrees do you have? 10. Do you belong to any organizations for recreation therapists or activities professionals? If so which ones? 11. Are you credentialed as a recreation therapist or activities professional? 12. Have you any training or certification as an occupational therapist? 13. With what populations do you work? (Geriatric, Rehab, Huntington’s Disease, Dementia, HIV/AIDS, etc.) 14. Do you plan to continue working in a nursing home for the next three years or longer? 15. Do you plan on moving on to a supervisory or administrative position in nursing home? 16. Would you like additional training or education for your work? Does your facility or union provide or pay for such training or education?
