Third party supervision/licensure

Hello all

I will be receiving outside supervision since the job I will be starting shortly

doesn't have a

licensed art therapist on sight and I working toward licensure. I was told that

there are no

standard agreement form and I must draft one myself.

Does anyone have suggestions on what I should include in the form and/ or know

where I

can obtain an example?

Just want to make sure I cover everything.

Good News for LCAT's:

Just received my application to renew my LCAT license from NY State. I've been concerned about what would be required, but the answer seems to be - nothing at all, except to fill in the forms and sent money.
