You'll get a variety of ideas of how folks are handling this question! We've

developed a sheet broken into 1/3s (PT, OT, ST) with a selection of what all

generally do with patients so as many as desired can be circled, a line

noting frequency of service, another circling option r.e duration (<1 wk, 1-2

wks, 2-3 wks), and lines for date of order/signature of therapist. Bottom of

page has lines for date & signature of physician & /or unit medical director

(some facilities develop the option where a physician can defer routine

medical care to unit physician) confirming acceptance of above

" recommendations " or orders for therapy.

We require that ALL therapy orders, sans the initial eval order, be noted as

clarification orders (they are, really...per continuation of an original

order for a particular service). Under PPS, this also assists the RNACs in

determining 'new' orders for RUGs purposes (Nursing as well has their own

'clarification orderrequirements).
