We've developed a sheet broken into 1/3s (PT, OT, ST) with a selection of what all generally do with patients so as many as desired can be circled, a line noting frequency of service, another circling option r.e duration (<1 wk, 1-2 wks, 2-3 wks), and lines for date of order/signature of therapist. Bottom of page has lines for date & signature of physician & /or unit medical director (some facilities develop the option where a physician can defer routine medical care to unit physician) confirming acceptance of above " recommendations " or orders for therapy. Guest guest Posted April 17, 2000 Report Share Posted April 17, 2000 We require that ALL therapy orders, sans the initial eval order, be noted as clarification orders (they are, really...per continuation of an original order for a particular service). Under PPS, this also assists the RNACs in determining 'new' orders for RUGs purposes (Nursing as well has their own 'clarification orderrequirements).
