" Replacement of gel with saline filled implants is an appealing choice to many women contemplating explantation. We expected that silicone envelope saline filled implant would be safer in that they lacked the potential immunostimulation of the gel-filled implants. " " Medical records of 14 consecutively recruited women who had replaced gel with saline impants were retrospectively reviewed. The mean age of this group was 45.5 years (range 32-68 years). Mean time of the onset of clinical signs and symptoms (post gel implantation) was 3.84 years.... For this sample, the mean duration of exposure to gel implants was 11.07 years, while the mean duration of saline implantation was 3.28 years. " " Of this group, the clinical status of 7 (50%) worsened, 4 (28.6%) remained the same, and only 3 (21.4%) improved compared to the previous study sample of women with a similar duration of gel implant exposure and a comparable severity of local and systemic clinical signs and symptoms wherein 70% improved and 27% stabilized. We conclude that the majority of women exchanging saline for gelimplants did not manifest a significant improvement in their clinical status and 50% actually demonstrated a deterioration. While data are preliminary, we currently recommend that symptomatic women who remove their gel filled breast implants not replace them with those that are saline filled.
