patient satisfaction survey re: nutrition counseling

Hi, I work for a home-delivered meals program,

federally funded, and do home visits with the elderly

for nutrition counseling. NYS is now requesting that

we use a client satisfaction survey to determine the

client's opinion of our home visits/nutrition

counseling. Does anyone have one that I may adapt

rather than having to reinvent the wheel? thanks. Sue Nichols
