Patients of Dr.Shoemaker, survey

Members,

I have been asked to assist Dr.Shoemaker's office concerning the VCS

testing, CSM and his protocol in general, since a good portion of his

patients are on this board. I am requesting at this time, if you would

please email me concerning this survey. This is NOT for an open

discussion on the board.

Please explain how his testing/protocol has helped you. Please limit

response to one paragraph, please. All information will remain

confidential.

I have received a couple of questions concerning this request. One

is about adding their name to the paper, the other is if more than

one person in the family is a patient.

This is a response I received from the coorindator as far as name:

" Whatever they are comfortable with. The first name and city, state

is good. If they want to add the first initial of their last name,

even better. If they want to be anon that is ok too, but less

desirable. "

As far as more than one patient per family, you can either do a

paragraph on each person or if it is easier to do it all in one

pargraph that is fine too.

Also, I think the age of children is important. Adults that is your

decision.
