Medication administration errors are something we've all run into on a

professional basis- however, this type of incident occurred in a personal

context as well. Last year, after surgery to perform 7 bypasses, my father

was given Atenolol as 'standard post-bypass medication'. This might have

been standard but for the fact that he had just been taken off a prolonged

Levophed drip to stabilize a dangerously low blood pressure. This resulted

in a second, unnecessary, emergency surgery to treat what the hospital staff

assumed was cardiac tamponade. Only an accidental comment by a nurse

alerted us to what had happened. The whole episode was explained away as

merely a charting error, due to the medication sheets being kept in one

location, and the patient charts in another, with no cross reference or

check between them.

>look for drug involvement in reported birth defects, heart

>problems and other common disorders that are frequently caused

>by prescription drugs. In the same way that the world's public

>health specialists aggressively seek out new strains of

>influenza, FDA needs to be aggressively seeking out new side

>effects of drugs.

>Rather than strengthening the U.S. government's drug safety

>programs, the present Congress has recently diminished the

>powers of the FDA to monitor drug safety. Congress now allows

>drug companies to pay fees which FDA uses to speed up the

>approval process for new drugs. As a result, during 1996-1997,

>FDA approved 92 new drugs for market -- twice the previous rate.

>However, Congress specifically prohibited FDA from using any of
