AND NEED OF IMMEDITE TREATMENT! neonate,immunicompromized patients-prophylacticfluconazole- progression from initial colonization to massive colonization to systemic infection. ? defination of systemic fungal infection(SFI)-defoned a positive culture(1)from blood(drawn from peripheral sites)or(2)from urine,sterile bladder catheterization(with growth of 10,000fungal organisms ml.)or(3)from CSF or (4) frpm inteavacular catheter tip(only if.....previous document SFI....... diagnostic implemented guidelines treatment,identifacation from cultures... analysis showed an assc. between occurance of SFI and colonization. fluconazile reduced or prevented proggression to infection in more than half of colonized neonates. most effective in reduceing or preventing proggression from colonization to infection. discusion- fluconazole wanes beyond a certain intencity of colonization, showing need for starting treatment early, not waiting until colonization is detected. studies show a reduction of morbidity and mortalitly,both adults and infants in this strategy of early preventive practice. http://pediatrics.aappublications.org/cgi/content/full/117/1/e22 Quote Link to comment Share on other sites More sharing options...
