My last 24 hour test showed a count of 5167 where 125/150 is normal. I do have kidney damage from Blood Pressure and Diabetes but other than leg swelling and minor aches and pains it isn't much of a daily problem. I do have to watch what I eat and monitor my BG's and BP more carefully than others so I can help slow down the process of Damage. My Doctor has recently taken me off of Adalat CC and Captopril as they may be causing some of the problem . He now has me on Lotensin and Cardizem CD Cap SA. It is his opinion we can decrease the 5167 # to at least 2500. I know this doesn't sound very good but any decrease is a decrease. I know it is scary to go through all of this but just remember it's just another bump in the road . The problems you are having can be controlled and lived with .. Find out what types of proteins they are testing for and how. Some test methods are not reliable for the types of proteins that are of most concern to diabetics. The kidney has several ways to prevent protein loss in the urine. Although healthy adults excrete 80-150 mg of protein in the urine daily, if a urinalysis shows proteinuria, further tests should be done to see if there is renal disease. Loss of protein in the urine is a sign of kidney damage. The first step would be to determine the quantity or amount of protein lost. This is done with a 24-hour urine collection. Protein excretion above 3.5 g/24 hours is termed massive proteinuria. This usually occurs when glomeruli (filters in the kidney) are damaged and allow plasma proteins like albumin to be lost. If you lose a significant amount of protein in the urine, you will develop swelling and high amounts of fats in your blood (hyperlipidemia). Massive proteinuria, edema (swelling), and hyperlipidemia is often called the Nephrotic Syndrome. Once you determine how much protein is lost, further tests can be done to determine the type of protein lost in the urine. Measurement of individual proteins can be done with urine electrophoresis. The predominant protein loss can clue one into the source of renal pathology. For example, large amount of albumin loss leans towards problems in the glomerulus. Large amounts of Bence proteins makes one think that there is not a problem with the kidney, per se, but an overflow of proteins. This can be seen in certain diseases, i.e. Multiple Myeloma. Your primary care physician should determine when or if you should see a Nephrologist. You should inform him/her if you are having problems with swelling. Guest guest Posted May 4, 1999 Report Share Posted May 4, 1999 Dawn - In January I had to go on insulin. I had been on glyburide and glucophage and was on maximum dosage with my blood sugars rising. My tests also showed albumin in the urine. After being on insulin since January, the endocrinologists had me take another test. My blood sugars had come down with the insulin and the albumin in the urine was gone. They said the protein in the urine could lead to kidney problems if not treated. I am very happy that the insulin took care of the problem and that it can be reversed. I have had diabetes for about 10 years now. I know it's scary. Guest guest Posted May 4, 1999 Report Share Posted May 4, 1999 Hiya Dawn, High protein content in urine can be a symptom of kidney damage. I know that I showed high protein on my last visit. I've been diabetic since Jan of 98. Well, my doctor put me on Avapro. It's an ace inhibitor, which is actually used for high blood pressure, but with diabetics prevents further kidney damage. Guest guest Posted May 5, 1999 Report Share Posted May 5, 1999 Jo - I too am on an ace inhibitor - Lotensin - originally for high blood pressure but since my diabetes diagnosis, I have lost some weight and the blood pressure went down some. I was able to cut in half the other two meds I take for the blood pressure but the doc told me he wanted me to stay on the ace inhibitor because of the benefits they say it is having for diabetics and kidney damage. I don't have any known kidney damage yet but hopefully this all is true and it will help or prevent the damage. He said he would probably keep me on that forever.
