The patient has known (s/l lel-tid or mel-tid or ???) diabetes mellitus which has left him with peripheral neuropathies. He really stumbles through this and I'm not certain if he's trying to correct himself or what. The only thing close that I can come up with is " latent " but would really be a stretch for me (as I re-listen) to pull this out of what he said! I don't think the patient's diabetes mellitus can be latent and known at the same time. I also think diabetes advanced enough to cause peripheral neuropathy is probably not " latent. " 2. I think your guess, that he started to say mellitus before diabetes and corrected himself, is a good one. I hope my conjectures help.
