FDA Adverse Event Reporting Form

FDA accepts online adverse event reports, but also allows paper reporting by mail using this form. The form gives you an idea of what the online report asks for, but it's easier to look at all on one page like this. Wouldn't let me copy the form and post it here, but here's the link....
