Guest guest Posted November 18, 2008 Report Share Posted November 18, 2008 My PCP has referred me to hospice care because of declining health and difficulty getting the in home services I need to keep me alive. So far, the hospice asked for a form to be filled out which the doctor did and then faxed them some information related to medical history and physical information. I don't know the timeframe for when they will approve my transfer and begin providing services and such but just wanted to let you know what is happening. Anyone else had experience with getting services from hospice? L; pulmonary fibrosis secondary to MS, autoimmune disorders, bird handlers disease and various mold allergies, exposures etc; along with a host of other co-morbid diseases.
