Here are some links I found for the evaluation and treatment of mitochondrial

dysfunction. The first article is probably the most complete in terms of

evaluation and treatment.

Markers that can be used include, total and free carnitine, pyruvate, lactate,

ammonia, alanine (specifically alanine:lysine ratio), AST, bicarb, creatine

kinase, and markers of oxidative stress. An organic acid tests could also give

an indication of mitochondrial function. I'm sure your thyroid function has

been extensively involved, but there can be some subtelties to that and I

usually like to check free and reverse T3 in addition to the usual TSH and free

T4 since some people (which frequently includes mold exposed patients) don't

convert T4 to T3 effectively and need some T3 support. A muscle biopsy can, of

course, also be useful, but is a bit invasive.

Also I do strongly recommend the Detoxigenomics study from Genova Diagnostics

and would add on an evaluation of MTHFR and Apolipoprotein E. Genova also has

a very extensive nutritional eval which includes evaluation of fatty acids,

amino acids, RBC minerals, vitamins and many other nutritional markers called

NutrEval that might help identify some things that could help you. That test is

a $1200 test that you can get for $149 through their Pay Assured program if you

have a PPO other than Aetna and a few others. Blue Cross and Blue Shield are

covered.

Let me know if I can help with anything and I truly hope things start to

improve for you soon.

http://www.icdrc.org/documents/Mitoandautism2008.pdf This is a very complete

article by Dan Rossignol that includes evaluation and treatment. It refers to

the ASD population, but really can apply to anyone with environmental illness

and mitochondrial dysfunction.

http://clinicaltrials.gov/show/NCT00327756 A study in bipolar patients

evaluating for mitochondrial dysfunction. Not that useful, but it included

using a skin biopsy for evaluation of mitochondrial function which is at least

less invasive than a muscle biopsy.
