If so, what precautions did you take when deciding about herbal treatment. I've had a look at the public assessment report from 2007 on the MHRA website which gives a fair amount of detail, however if anyone could shed light on any or all of the following, I would be grateful. 1) What is the mechanism behind the purported interaction of antacids and hydroxychloroquine and could it be relevant more broadly to herbs with an antacid action? 2) Hydroxychloroquine appears to be a prodrug, with the liver metabolites being the active constituents. According to the MHRA document it is excreted predominantly via the kidneys. However, given that the drug accumulates over time and several herbs influence liver metabolism, I'd appreciate any further thoughts on combining herbs with this drug? 3) Given the well-documented risks to eye health of taking this drug, can anyone shed light on the mechanism and protective strategies, if there are any? The patient in question is mother of two in her mid-thirties, diagnosed with SLE in her mid-twenties. She's currently taking hydroxychloroquine sulphate 200mg bid, oral steroids 8.5mg per day and calceos (calcium and calcipherol).
