Hello,I'm thinking of using intake forms with my new patients to give me a head start and save time. I was wondering who's already using them, what's on them, and what difference they think it makes to their practice?. If anyone could send me an example I'd be really grateful. Thanks very much. I have a couple. Contact me offlist and I'll send them.I do find they save time and all sorts of oddball questions are dealt with. One I find most useful as the questions are grouped by systems, and a glance shows patterns quickly.No form is a replacement for a good case history though! You might also want to look at S.O.A.P. notes too, currently enabling a template of these to be attached to my practice management software for those who like to use them and as part of clinical audit tool. They give a quick outline of condition, diagnosis treatment strategy etc. Many descriptions on web, popular with US users.
