PROVIDER AUDIO TELECONFERENCE

" DISPELLING ANTI-VACCINE MOVEMENT FALLACIES "

The New York State Department of Health Immunization Program will be

conducting a statewide audio teleconference entitled " Dispelling

Anti-Vaccine Movement Fallacies " on May 22, 2001, 12:15pm - 1:15pm and

repeated on May 24, 2001, 12:15 - 1:15pm. Nationally known speakers, Drs.

Sharon Humiston and Judelsohn, will discuss common immunization

fallacies and evidence-based information to refute these fallacies. The

teleconference will be one hour in length and will include a question and

answer segment.

Dialing 1-888-391-0089 on either date can access the teleconference. To

ensure that enough telephone ports have been reserved for this audio

teleconference, you are requested to complete the registration form below

and fax to (518) 474-1495 by May 15, 2001. Failure to do so may result in
