The question for her is perhaps , how much is prepared to increase versus the side effects, and for how long. Well, and before that, Is it working? It might be that a high dose is effective for a short period of time, gets her through the acute episode which quickly resolves , and then over to you for the wider picture of why the episode occurred, preventing future ones and addressing the fallout of a short burst of toxic medication. Chances are it won't be like this, the higher and higher does of Carbomazapine won't touch it, and the acute condition will get 'stuck'. Then you have to work out strategies between you. The thing to be clear is which herbs potentate adn which lower the dosage. Hypericum is likely to fall into the latter categorie, so it is not increasing the likeyhood of toxicity, just counter productive if she is exploring the efficacy of increasing the dose. There may be an issue of photo sensitivity - but well it's hardly the weather for it! For that reason it may be Ok to include as a specific anti-herpetic. An approach in this instance would be to carefully introduce the herbs that you think appropriate to the person , and monitor , monitor, monitor. Am giving my patients scoring charts, more and more lately, for loads of things, but is especially helpful in relation to pain. But it could also include anticipated side- effects, interactions. And there is always the possibility of really focussing on topical preparations, initially or ongoingly. Something like gelsemium, Liquorice, capsicum, hypericum.(careful with the capsicum) Perhaps in the form of tinctures painted on..
