I had been there before and you could print out a form unfortunitely before I sent the form in our server pissed us off and we switched so now I want to register, but i need a noninternet way. The pigmentation is purely a cosmetic effect, and may get worse if it is due to the Minocycline. You can send a check. Details are on the form. I have had PC problems for a few months which has made it difficult to process any internet based requests.
