That does not come within the remit of the ICO. They asked me if I sent letters of referral. If they are done on a computer, that in itself is sufficient to trigger the need for registration. I specifically asked them about colleagues who may be acupuncturists or herbalists. The same applies, only paper records are fine, anything on a computer makes a need for registration.
