Transplants are covered only at VA facilities but they will pay for the drugs as long as the pt is established with a physycian at their closest VA facility. We have had patientstransplanted here, different organs, and have never had an issue. VA usually wants a call from the transplant Coordinator a few days before the hospital discharge, to coordinate the medications. My understanding re: patients with VA who are medically managed through a VA physician,  is that we would need to get authorization from the VA physician to do the transplant at our hospital versus having the transplant at a VA hospital. How does that work at your centers? Is the TFC responsible for setting up a discussion with the VA physician? I would imagine it would really need to be physician to physician, or at least clinical coordinator involvement. Is that correct? M. Thanks for the information. My scenario is a patient with Medicare and VA benefits. Obviously, if we do not get permission for the transplant from the VA, then the patient essentially has Medicare only. That is where my dilemma comes into play. It is great to know that we could do the transplant under Medicare and have him obtain his immunos through the VA, but the issue of no secondary insurance still remains. M. We have never gotten permission for someone to utilize their VA benefits here. They require them to go to a VA facility. I have the VA patients sign a “Letter of Understandingâ€.  The letter advise the patient if you choose to have the  transplant at our hospital we will bill Medicare but you are responsible for the  Medicare part A deductible, as well the 20% balance after Medicare pays on all covered charges.  Of course, I work with the VA transplant nurse to ensure the post medications can be obtained at a VA center.  We’ve transplant a lot of VA patients with no financial or medication problems.   Thanks From: TxFinancialCoordinators [mailto:TxFinancialCoordinators ] On Behalf Of lisa.christiansen@...Sent: Monday, June 18, 2012 10:51 AMTo: TxFinancialCoordinators Subject: RE: Re: VA and transplants Thanks for the information.
