RE: patients with grants

Good Morning. we give the patient any information we think they can use on the pre end, and they get signed up for programs such as Chronic Renal Disease Program (here in Pa) by their dialysis unit. On the post end, it is the SW who gets them signed up for Healthwell, PAP's, grants, etc.

Good morning, everyone.I am curious to hear from other TFCs what their estimated percentage of patients receiving assistance is - either from Healthwell, PANF, or any other organization. Also, who keeps up with the grants - ie submitting requests for reimbursement either to the patient or insurance company for premium assistance patients. Is it the TFC, social worker, or other hospital/office staff?Thanks,

I work with HealthWell a good bit. I have the patient call and they are responsible for all grant work. I simply complete the physician statement of treatment and fax it with any documentation. The numbers of patients required to meet a fundraising goal is climbing rapidly here. Other than that, at this time, in general, we do not accept PAP as a form of financial support because in our experience, neither patients nor their referring MDs maintain it reliably after the patient graduates at 90 days post, and we do not have the staffing within the transplant program to take it on except in emergency situations. Some programs, like Nazih Zuhdi in OK, have impressive systems for keeping pts enrolled in RX PAPs.
