Medicare COB at time of transplant

Hello everyone,

I was just on the phone with a patient and his insurance company together and the Case Manager stated that Medicare automatically becomes primary at the time of transplant in spite of the COB period. I thought the transplant event does not change the COB and that if it has not been 30 months Medicare will remain secondary until the 30-month mark. Any thoughts??? I would greatly appreciate your help on this one!
