Anthem BCBS and IBM -

Have a patient with an Anthem plan – BCBS of New York with

IBM (they changed from Empire BCBS to Anthem on 1/1/12). I faxed the standard

benefits request form to Anthem which called me back and said that this case

was not handled by them but is handled by Case Management in New York. I have spoken with case

Management which is nurses and they don’t do transplant benefits. I

have received general benefit information and would like to confirm his

transplant benefits – do any of you have a phone number for such?
