Large Granular Lymphocytic Leukemia

We have a patient with LGL Leukemia and she is having an allogeneic related stem cell transplant. We are trying to get approval from BCBS of MS, but they cannot

find a medical policy to attach this diagnosis to so they can approve it. Has anyone transplanted a patient with this diagnosis? If so, did you have trouble getting it approved?
