When a patient has insufficient insurance or is unable to be seen at your

facility and you have to close out the patient, in your center who would write

the close out letter to the patient? In our center we only contact the patient.

Could you also provide me with a copy of the closed out letter due to these

circumstances for our records?

I appreciate any help on this.
