Per our social worker a CMS2728 was completed in 2010 with applying for ESRD checked yes. The patient believed he was on it, therefore seemingly wanted it. My research did not find Medicare effective for him ever and the dialysis center confirmed he is not on Medicare as well. Should we have him apply again? Do we have to complete another 2728?
