Guest guest Posted June 5, 2012 Report Share Posted June 5, 2012 I use a separate form from their medical benefits....as stated I try and give the best guestimate to the patient at the time the prescription was processed.....its cut and dry for some patients, but patients with Part D it is always harder. It is just n excel spreadsheet that the pharm d help me with. I explain that this form is ONLY about money and that it shouldn't be used to know what to take when to take.
