Good Morning,

I have a patient 50 yrs of age who is

currently NOT enrolled in Medicare, however we are a Medicare approved

facility. He’s concerned when he retires Medicare will not pay for Immunos

for his potential kindney transplant considering Medicare didn’t pay for

the transplant. He informed me he needs to fill out a 2728 form in order to

receive Immunos down the line when he retires. Im not familiar with this 2728

form. Could someone possibly shed some light on this situation? Thanks
