When calling to get preauthorization for Initial Evaluation, I use 50360 for the kidney transplant. The diagnosis code I use is 585.5 is not on dialysis or 585.6 if they are and V42.0.

There isn't one CPT code to use for the recipient evaluation. You need the codes for each individual test that you perform. The 50360 is the transplant CPT we provide when obtaining auth for the actual transplant. We will send a list of all the testing required to the plan so they can approve.
