Medicare coverage for Hep B series

Hi all, must the kidney failure patient be at Stage 6 for Medicare to cover the Hep B series? Or will they accept 585.4 or 585.5 for medical necessity if pt is pre-dialysis and pre-transplant? Thanks for any feedback,
