We do not meet with potential donors at UVA. We have a donor coordinator who explains the complete donor process.

Hi ,

Here at ETMC the living donor coordinator and social worker do most of the

interview. I only meet with them for a few minutes to explain the financial end.

Hi Kim,

We are currently implementing a financial clearance screen for the donors.

Our plan is to run donors insurance (if they have any) and with the donor coordinator, meet with the donors explaining the billing process (because we don’t want them to get billed for any donor related services,

and inform them of the post follow-up as well).

We are finding that the donors are left in limbo – and with the shortage of organs, and long wait times, living donors are becoming more prevalent.

Once we have a system in place – we will definitely share!

Why would you run the donor's insurance? Is it just in case something is found

in your workup the needs treatment?

Yes, our physicians are concerned for the donors. What we don’t want is to find something (i.e., abnormal ct) and say “ follow-up with your pcp” when they don’t have one, nor insurance to assist them. It’s really

a slippery slope and morally/ethically not safe.

I guess we would like to be able to sleep at night!

That is what we are “discussing”. Currently – the donor’s financial situation is not in the decision making process – but the “what if’s” are troubling – and that is why we are revamping and trying to make it

more uniform.

I – personally – feel that is why the financial coordinators/counselors need to be in the mix of the screening donors - should the potential donor not have insurance – the coordinators/counselors can direct the

donor to sources (i.e., Medicaid, ICHIP – for Illinois pts-, private policies, etc). COUNSEL them, just as they would the recipient.

My understanding – is that the regulatory bodies (CMS, UNOS, etc) are heading in that direction, and will require the same services that we provide the recipients, we provide to the donors.

Very well said Joanne. Our liver

program requires donors to have insurance. We have had “findings”

of illnesses on donors in the past which opens a whole can of worms for them

for possible pre-existing condition ( in some states ) and for any future life insurance

etc. I meet with all liver and kidney donors and go through this with

them. I am a big advocate of donors having insurance.

That is what we are “discussing”. Currently

– the donor’s financial situation is not in the decision making

process – but the “what if’s” are troubling – and

that is why we are revamping and trying to make it more uniform.

I – personally – feel that is why the financial

coordinators/counselors need to be in the mix of the screening donors -

should the potential donor not have insurance – the

coordinators/counselors can direct the donor to sources (i.e., Medicaid, ICHIP

– for Illinois

pts-, private policies, etc). COUNSEL them, just as they would the

recipient.

My understanding – is that the regulatory bodies (CMS, UNOS,

etc) are heading in that direction, and will require the same services that we

provide the recipients, we provide to the donors.

Yes, our physicians are concerned for the donors. What we don't want is to find

something (i.e., abnormal ct) and say " follow-up with your pcp " when

they don't have one, nor insurance to assist them. It's really a slippery slope

and morally/ethically not safe.

Our plan is to run donors insurance (if they have any) and with the donor

coordinator, meet with the donors explaining the billing process (because we

don't want them to get billed for any donor related services, and inform them

of the post follow-up as well).

We are finding that the donors are left in limbo - and with the shortage of

organs, and long wait times, living donors are becoming more prevalent.

Once we have a system in place - we will definitely share!

So, would it be better that the uninsured potential donor be denied, but not

have had the cancer or whatever found? You are right, this is a very troubling

Personally, I don’t think the donor should be denied, but that is the current “discussion”

I believe the donor should be counseled by the Financial Coordinator/Counselor to obtain insurance – whether it be Medicaid, Private Insurance, etc., in the event of an issue, and to just have basic coverage.

We are fortunate to have a County Facility less then ½ mile away which handles all uninsured patients, and that’s where we would refer the donors, but again – once they are “happy” with the services at our facility-

transferring them to a County facility is difficult.

I believe we are only touching the tip of the iceberg regarding living donors, and now with the National Registry – it’s bound to become a real issue – and I see the regulatory bodies getting full hands on involved!!

Who said transplant is boring??

Yes, our physicians are concerned for the donors. What we don't want is to find something (i.e., abnormal ct) and say " follow-up with your pcp " when they don't have one, nor insurance to assist them. It's really a slippery slope and morally/ethically not safe.

Hi Kim,

We are currently implementing a financial clearance screen for the donors.

Our plan is to run donors insurance (if they have any) and with the donor coordinator, meet with the donors explaining the billing process (because we don't want them to get billed for any donor related services, and inform them of the post follow-up as well).

We are finding that the donors are left in limbo - and with the shortage of organs, and long wait times, living donors are becoming more prevalent.

Once we have a system in place - we will definitely share!

I think the main thing is consistency.  If you require your

donors to have coverage before you start the work-up, then all of your donors

should have coverage.  We, for example, require our liver donors to have

coverage with a specific benefit level.  Our kidney program, on the other hand,

does not require our donors to have coverage. 

The coverage really should be to protect them should you find

something outside of transplant services.

Personally, I don’t think the donor should be denied,

but that is the current “discussion”

I believe the donor should be counseled by the Financial

Coordinator/Counselor to obtain insurance – whether it be Medicaid,

Private Insurance, etc., in the event of an issue, and to just have basic

coverage.

We are fortunate to have a County Facility less then ½ mile

away which handles all uninsured patients, and that’s where we would

refer the donors, but again – once they are “happy” with the

services at our facility- transferring them to a County facility is difficult.

I believe we are only touching the tip of the iceberg

regarding living donors, and now with the National Registry – it’s

bound to become a real issue – and I see the regulatory bodies getting

full hands on involved!!

situation is not in the decision making process - but the " what if's "

are troubling - and that is why we are revamping and trying to make it more

uniform.

I - personally - feel that is why the financial coordinators/counselors need to

be in the mix of the screening donors - should the potential donor not have

insurance - the coordinators/counselors can direct the donor to sources (i.e.,

Medicaid, ICHIP - for Illinois pts-, private policies, etc). COUNSEL them, just

as they would the recipient.

My understanding - is that the regulatory bodies (CMS, UNOS, etc) are heading

in that direction, and will require the same services that we provide the recipients,

we provide to the donors.
