Hello all,

Here is the conference information again ;0) I know it is fast approaching but

the hotel has reassured me that they will most likely have rooms available at

the quoted rates even after the extended cut off date IF you give the

reservations desk the Still’s Conference ID # that is in the email below. Come

if you can but if not able we will miss you and hope to be able to bring back

information!

J. Cush, MD Professor of Medicine and

Rheumatology Baylor University Medical Center

J. Cush, MD, received his undergraduate degree from St. s University

and his medical degree from St. s University School of Medicine. After

completing an internal medicine residency at Coney Island Hospital in New York

City, Dr. Cush finished his rheumatology training and fellowship at University

of Texas Southwestern Medical School and Parkland Memorial Hospital. He has been

on the faculty at Southwestern Medical School since 1987, where he is a clinical

professor of internal medicine. He is the director of clinical rheumatology for

the Baylor Research Institute and a professor of medicine and rheumatology at

Baylor University Medical Center in Dallas, Texas.

Dr. Cush is devoted to patient care, medical education and novel drug research

for the rheumatic diseases. He has been published in more than 100 medical and

scientific publications and has written two textbooks on rheumatology:

Rheumatology: Diagnosis and Therapeutics (2nd edition, Lippincott, &

Wilkins, 2005), and Rheumatoid Arthritis: Early Diagnosis and Treatment (2nd

edition, Professional Communications, 2008).

Dr Cush is the recipient of numerous awards and distinctions, including the

B. Hingson Humanitarian Award (1988), Intern of the Year (1985), Teacher

of the Year (1999), Best Doctors in America (1996, 1998, 2001-2002) and Best

Doctors in Dallas (1998-2007). He has held numerous leadership positions for the

National Arthritis Foundation and the American College of Rheumatology and has

served on the Arthritis Advisory Committee for the Food and Drug Administration.

ADDED EVENT DURING CONFERENCE

We were waiting until arrangements had been made to let you know about this

exciting event that will be taking place at the conference this year. Dr. Cush

has worked with a medical reporter named Hammarley who has produced videos

for his practice website and social media. Because of the generosity of a few

people and Mr. Hammarley, he has offered to use his video production company to

make 2 or 3 short videos (3-5 min) about Still’s disease, 1) what it is; 2)

the difficulty in getting diagnosed & 3) advances in treatment. It is planned

for this to be done from chosen patients in attendance at the conference. These

would be used to promote the web sites and give disease information through the

social media (Facebook, Twitter, You Tube, Medical Publications) as well as

raising public awareness for research, etc. Dr. Cush is also working on another

project to present at the conference. We will inform you if it works out in

time.

Reservation Details

Orleans Hotel & Casino

4500 W. Tropicana Ave. Las Vegas, Nevada

ROOM RATES:

The hotel has extended us FIVE rooms for Saturday, September 1st check in at a

discounted rate of $ 99.00 and must be booked through the booking number listed

below. The rate for Sunday night, September 2nd, is offered at $ 39.00 and

Monday, September 3rd through Wednesday night, September 5th will be offered at

$ 29.00 per night. ALL with a minimum TWO night stay.

Please be advised that all rates quoted are net, per room, per night, allowing

for single or double occupancy. Any Deluxe rooms requiring triple or quad

occupancy will be surcharged an additional $ 15.00 per guest, per room, per

night. (Children age fourteen and under, in the same room with parents in

existing bedding, are free.) All rates are subject to a County room tax,

currently twelve-percent (12%). This tax is subject to increase without prior

notice. Reservations for dates other than those specified above will be accepted

on a space available basis as to accommodations and room rates.

RESORT FEE:

There is a mandatory Resort Fee, currently $ 6.00 per room, per day, allowing

access to the Fitness Center, in-room coffee, unlimited local & 800 calls as

well as free Wi-Fi in Java Vegas Coffee Shop & High-Speed Internet Access in our

Business Center. The Resort Fee is subject to increase without prior notice.

CHECK-IN/CHECK-OUT:

Check-in for individual arrivals begins at 3:00 p.m. Check-out time is at 12:00

noon. Hotel agrees to make rooms available prior to the stated check-in time and

to extend late check-out to guests on a preferential basis, if available.

GENERAL HOTEL POLICIES:

Guestrooms must be occupied and registered to at least one adult, who is

twenty-one years of age or older. PHOTO IDENTIFICATION WILL BE REQUIRED UPON

CHECK-IN. Hotel reserves the right to refuse or terminate service to any

individual(s) when not in compliance with this policy.

Upon arrival, all guests will be required to post a $ 100.00 security deposit

via a credit card to secure their individual room accounts.

The number of Non Smoking rooms at the Hotel are limited. As such, although

Hotel will make every attempt to accommodate requests for such rooms, Hotel

cannot guarantee availability.

RESERVATION PROCEEDURES:

Prior to cut-off date, individuals are encouraged to book their reservations

on-line. To receive your special group rate members will need to enter the

assigned Reservation ID: A2STC09 through the Hotel’s website at

www.orleanscasino.com/groups .

Prior to cut-off date, individuals may also call Hotel’s Room Reservations

Department direct at and identify themselves as members of the

International Still’s Conference. Without the group name you will not receive

the special rate as noted above and may be advised that the Hotel is sold out.

Operating on Pacific Time, hours of operations are Monday through Friday from

6:00 am to 11:00 pm; Saturday and Sunday 7:00 am to 11:00 pm.

The equivalent of one (1) night’s room and tax will be required in either the

form of a credit card with the expiration date, company/personal check, American

Express; Diners Club; MasterCard; Discover; VISA; JCB or Cashier’s Check.

Hotel accepts company/personal checks for payment of deposits only and must be

received a minimum of thirty (30) days prior to arrival date. CREDIT CARDS WILL

BE CHARGED IMMEDIATELY BY HOTEL TO SECURE THE FIRST NIGHT DEPOSIT.

CUT-OFF DATE: HAS BEEN EXTENDED

The cut-off date has been extended for room reservations and will be 08/17/12 .

The Hotel will make every attempt to accommodate last minute or late

reservations at the prevailing rate on a space availability basis.

CANCELLATION POLICY:

Hotel policy for cancellation of an individual guaranteed reservation is at

least seventy-two (72) hours prior to scheduled arrival date and must be

received in writing. Rooms released less than seventy-two hours prior to arrival

date and all no-shows will be subject to a cancellation penalty equivalent to

the first night’s room and tax.

ACCOMMODATION OF DISABLED PERSONS:

The Hotel shall take all reasonably necessary steps to ensure the accessibility

of its facilities to the disabled attendees and to accommodate their special

needs. Please advise Hotel prior to arrival, of the anticipated needs for such

accommodations. A limited number of refrigerators are available upon request and

it is suggested attendees bring coolers if convenient for medications. The Hotel

does have scooter rentals available.

EVENTS SCHEDULE

1.. Sunday Evening Meet & Greet – 7:00 to 8:30 pm Upstairs in Conference

area lounge. Elevator is available.

2.. Monday is Conference Day – Check in at front table. Offered will be

Pastries, Coffee, Tea and Juices 9:00 to 10:00 am in Salon I upstairs. ($20. per

person donation requested at the door)

3.. Opening welcome by Treasurer/ Bob Himes 10:00 to 11:00 am

4.. Speaker Dr. J. J. Cush

11:00 pm What is Still’s disease and how is it diagnosed (the history of the

disorder, key critical features, how to assure the diagnosis)

WITH BREAK

12:00 pm Treatment of SoJIA, AOSD (treatment of systemic disease vs. chronic

arthritis, how to find the right MD to treat AOSD)

Lunch 1:00 pm – 2:00 pm

Gift drawing fundraiser 2:00 pm (A gift donation is appreciated) Tickets $ 1.00

ea.

Afternoon session to close by 4:30/5:00 pm

Some times during the conference may vary throughout the day as there will be

extra activities. Please tell me that day or before by email if I can

accommodate any special needs. Thank you.

Optional DINNER 7:00 pm A restaurant will be chosen by the afternoon of the

conference, if not before. Some do not take advance reservations.

Individual dinner tickets. Dress: Evening wear as comfortable.

Many guests stay over for additional days and take advantage of the Hotel

amenities and or activities around town. You may request the Hotel to send you a

brochure of what they have available when you make your reservations.
