Event: Celiac Disease Golf Tournament

Hello,

We are quickly approaching September 15th which is the date of the tournament. 

We would like to complete all registrations by this Friday, 08/31.  We still

would like to get about 10-20 more golfers (5-Foursomes).  Please share the

tournament information with all golfers that you know and encourage them to

participate.

Here are the registration options

1.  Fill out page 4 of the attached document, input a credit card number, scan

and email or fax it back and we will process the payment and send you a

confirmation. Fax # .

2.  Pay online at the following link and be sure to select Golf Tournament from

Choose Project: and for Messages: type in the option you want (i.e. Type of

Sponsorship, Individual Golfer).  You will also need to fill out page 4 with the

golfer information and send it back.

3.  Mail a check and the completed page 4 to the following address
