documentation of treatment times

I am looking for resources that require documentation of treatment time,

beginning and ending, for an acute care facility, in-patient rehab, and long

term-acute care facility. Are the requirements for outpatient setting different

than those for acute care in-patient and Acute care in-patient rehab?

Any guidance is greatly appreciated.
