RE: billing in an MD office

I used to see outpatients in a MD office a couple years ago and learned

the billing process by trial an error. I did both individual

appointments and group appointments and was reimbursed for both. There

are nutrition billing CPT codes for individual assessments and group

assessments. I don't have those right in front of me, but I am thinking

they were 97802, 97803, etc. You also billed in units to represent the

length of time spent with the individual or group. I billed using the

appropriate CPT code and of course the billable ICD-9 codes (diagnosis

codes) under the doctor's office location, but used my pin number (at

the time) or the new NPI number that all providers must have to bill.

Any registered dietitian can get his/her NPI number at

https://nppes.cms.hhs.gov/NPPES/Welcome.do. In other words the payment

was made to the MD office; and I was paid as an employee of the office

since that's what I was. I am almost sure you can bill using your NPI

number, but that number must be attached to a specific practice

location; therefore the payment would come in your name at that

location. By the way, no vitals were taken and the individual assessment

and group classes were done when the MD wasn't present. If you would

Be sure to double-check this, but I don't believe the MD can bill at all

for this if he/she is not providing any services, him/herself. So, your

best route would be to charge patients directly and have them pay out of

pocket. You may need to then provide the doc with a percentage, in

exchange for the use of services (building, materials, etc).

Make SURE you draw up a contract before you start offering any services,

though.
