Especially with the MTX because the medication is so thick and oily, otherwise you don't receive the full dose, always a little bit remaining in the end of the syringe and the length of the needle. Also, the MTX tends to stain the tissue and will look like a yellow bruise. I want my full dose! The air will not harm you. In fact, bubbles or small spaces of air in an IV tubing is quite harmless. While you wouldn't intentionally inject air into your veins, the studies show that if an entire length of IV tubing is full of nothing but air rather than flushed with fluid and all of the air goes into your veins, it still won't hurt you.
