I don't know about your state.... I am guessing they are all the same. You

need written permission from your Dr. Your Dr. will have the paperwork

needed. He/she fills it out, you sign it, take it to MV and they will give

you the permit.

There are certain requirements you need to meet. Your Dr. will know if you

meet them.

I JUST RECEIVED MY DISABILITY NOTIFICATION AND I NEED TO KNOW

HOW TO GET THE HANDICAPPED STICKER FOR MY CAR. I KNOW I NEED

TO GO TO THE DMV, BUT DO I NEED SOMETHING SPECIAL FROM MY DOCTOR ?

I went to my state DMV website and download/printed the form they have

for the plate/placard. The form required a physician signature,

basically prescribing it to you. Then just took it up to my county court

office (where we get out license plates) and they gave me my handicap

plate AND a placard for when I am in another vehicle. Of course when you

get new plates, you also get a new vehicle registration. They gave me an

extra copy of the registration and told me to have it on me any time I

am in another vehicle and using my placard, as that registration is the

proof that the placard is assigned to me. It was all quite simple. Good

luck on getting yours. I was very embarrassed at first, and still get a

little uncomfortable parking in handicap spaces, but I tell you, it

really makes a world of difference, much more then I had anticipated.
