Guest guest Posted November 8, 1999 Report Share Posted November 8, 1999 My understanding from our fiscal intermediary is that the 700/701 forms have never been required. You might do well to check with yours to learn what other formats are acceptable. At this home health agency, we do our part B plans of care on the same form that we use for part A (HCFA 485), with a 30-day certification period, and the homebound certification crossed out when appropriate. This has allowed us to stick with just one process form POCs. Hope that's useful.
